Integrated Physical Therapy is pleased to announce the option to schedule virtual office visits that allow you to receive assessment and treatment from the comfort of your own home thru a secure online video chat with one of our four therapists.

All TeleRehab visits must be scheduled thru our reception staff at 608-658-5352.

For your scheduled appointment, please click on the link to your therapist to arrive in the virtual "waiting room"

Before your appointment, complete the TeleRehab consent form and send to Integrated Physical Therapy following the instructions at the bottom of the form.

To make the most of your virtual appointment:

• Ensure your camera is on and that you are allowing video.
• Ensure your device is connected to a strong/stable internet connection. If using a cell phone, being connected to an internet source can provide a better connection.
• Connect approximately 5 minutes prior to the start of your appointment.
